php_printer.dll is rather an old extension. You have to use PHP 5.6 that you can set up automatically in ExeOutput by running the Web Update utility (in PHP extensions, near the PHP selector, click Download more PHP versions, and choose PHP 5.6 in the dedicated web update app).

Link to the PHP 5.6 printer extension DLL that is compatible with ExeOutput:

https://github.com/gimjudge/php/raw/master/php_printer%20-%20php%205.6.35_nts%2032bit.zip

Just unzip the PHP_PRINTER.DLL to the corresponding “ext” folder (PHPRuntime56 folder).
Close ExeOutput. Restart it.
Open your ExeOutput project, go to PHP extensions and you should see the Printer extension.
