Software EXE Icon Missing - SOLVED

Sorry for such basic issue but have spent last hour trying to solve without success.

I have read:

http://www.exeoutput.com/help/resources?s[]=application&s[]=icon

But no matter what I do my icon will not appear in compiled EXE. It stays with the default PHP icon?

My icon shows on the loading screen and also in title bar when software starts but not on the actual EXE file.

How the heck do I solve this?

Solved. Moved to using Windows 7. Issues seems to be Windows 10 machine.

It’s possible that Windows caches the icon for faster display. You need to refresh the icon cache in that case. See http://www.tenforums.com/tutorials/5645-icon-cache-rebuild-windows-10-a.html to force a refresh.
